This is a wonderful quality Progress Soap and H&P notebook progress note &and Medical History and physical note book . I got this for my daughter that's getting ready to start medical school. It has 75 templates with perforations, 2 page view . Each template consists of 4 page view . It has a hard plastic cover , perforated pages and premium non- spiral wire bound compact notebook measures 8.5"x5.5" and will fit into any white coat pocket or scrubs pocket .
